Post form data to email

I would like to make a form that puts the data in my email address.

I found the following code and it works fine, but I want to remove unnecessary fields. For example, how can I delete the Last Name and Email fields without breaking the working code?

After clicking the "Submit" button, nothing happens. No email, no messages. Just clear the page.

Just delete all links to fields that you do not want. When you get an error message, it's time to ask a question.

Also add a bug report during debugging, put this line on top of the script:

error_reporting(E_ALL);
