Create a table of contents

I posted this a couple of months ago on the Mathematica newsgroup, but didn't get a helpful answer. I thought I would try it.

The question was: I seem to be unable to find a way to generate a table of contents for the Mathematica Document that I am working on. Does anyone know this shelter feature?

David Annette pointed me towards AuthorTools, the old v5.1 which is still hidden in Mathematica. However, this does not work on my document (v7). Any clue?

Edit
The TOC must contain the correct section numbers (if they are in the style sheet) and the page numbers of the list (for this you need to consider the page size settings).

Perhaps look at the package code of Yuri Kandrashkin. The sidebar will be useful?
