There is a class that I modify that another author wrote. I am wondering how to correctly add my changes to the documentation, making sure that the original author provided the proper credit. Should I reset version control or continue it? Should I add PHPDoc to the original comments or create my own separate block?

It seems to me that you are asking how you should update the information value of docblock, and not record that you made changes. If so, just add more information to the docblock description and add new @ tags, if applicable.
