Dynamic database design?

im got a hard copy of the form that the hospital uses to collect information about its patients! the form is divided into sections, and each section has MANY YES / NO.

For example, one of the sections is the "History" section, in which there are more than 20 unconnected yes / no fields:

smoker (yes or no)

diabetes (yes or no)

Chronic lung (yes or no),,,,.

Another section is the stress test data section, which has the following questions:

Standard test (yes / no), if yes, what are the results (negative or positive), if positive (to what extent (low or high or medium)

...

I am invited to create a database, show the relationship tables and the relationships between them !: S: S it looks NOTHING like the "student-class database" or the "CD database" I came across! it's just .. i don't know .. DYNAMIC: s: s

I don’t have NO IDEA, how to even start developing this database or what things I should read or study or practice to create a database for such data!

PLEASE, HELP!
