Create a dynamic sitemap in SharePoint

Question

I developed a publishing portal at sharepoint.I have a requirement in which I need to create a sitemap for the entire web application.

This should be dynamic, in the sense that when we update the contents of any page in our web application, it should be immediately reflected on the page of the site. What are the possible ways to achieve this goal and which is the best possible solution, given the scalability and ease of setup?
