Migrating Oracle Data (and Index) to SQL Server - Data Capitalization

I am trying to transfer data from an Oracle database (10g) to SQL-Server (2008). I also want indexes recreated by SQL-Server. However, in Oracle there is a primary key defined for the first two fields, and it has the following data:

VALUE3   FOO4
VALUE4   FOO8
Value4   Foo8

When I get this data on SQL Server, it will not do this index due to data duplication. Oracle considers this case and considers that the 2nd and 3rd records are different.
