It is 16x16. If you create a .ico file that supports sizes 16, 32, 48 and 256, you will be closed.

I believe that it was always 16x16, but at some point between 98 and XP (maybe it was CE versus NT?) They learned to support full color and alpha transparency. If possible, try including the 256 color pallet version in ICO for safe mode, if not older versions of Windows.
