Using Formtastic to select from a set of radio stations and possibly create a new record

Question

I started using Formtastic in the last couple of days and came to my first charter.

I work with items that have several linked accounts. On the page you are interested in, you need to select the recipient account. This can be either an account that we already know about ("Primary Contact"), or a new account that you can create by filling out some information.

Here is the project I'm trying to replicate:

Using Formtastic, it is very simple to display a form for entering new account information.

  <%= form.inputs :street, :city, :for => :address %>

It is also very simple to create a radio accessibility selection list of available accounts.

  <%= form.input :address, :as => :radio, :collection => @addresses %>
