Why doesn't IntelliTrace show code?

I am debugging an application on an Azure server using IntelliTrace. Sometimes I see the code that threw the exception, but sometimes I can't. And the exceptions that the code I don't see are always the same.

Is there a reason why I keep getting the same message (no source) in a new tab when I double-click an exception to start debugging it?

I already know what exception is what I need to do to solve it, but I also need to know where to find the code! Reference.
