How do teams fit the current sprint lag elements?

Scrum and agile say that elements in the current sprint gap should be ranked in priority order and one element at a time by the whole team.

In practice, this never works for our team. Or the element is too small for all team members to be productive (including taking into account pairing). So we end up possibly doing two or three elements in a team at any time.

I would be interested to hear how other teams do it, as well as how many items they usually complete in a given sprint.
