Difficulties with distributing ad-hoc for testing on iPhone

I am trying to make ad-hoc distribution of my (first) iPhone application to a small group of volunteer testers. I looked at Apple's documentation, as well as a few blog posts, but I still have problems. I have a couple of questions about things that are not clear (at least for me):

When creating requests for certification of development and / or distribution for Common Name, should I use my name or the name of my company? I signed up for the iPhone developer program as a company, and the name of the company is displayed on the portal, but also my own name as "Agent."

In addition, the Apple documentation (Publishing Testing Applications section of the Developer's Guide) contains a diagram showing the tester’s training profile containing information about the tester’s device, test application ID, and development certificate. When I try to create a Provectoring Tester (Ad-hoc) profile in the portal, it selects a distribution certificate, not a development certificate. It is right? This seems to make sense, but does not fit the chart.

Any other recommendations for special resource provisioning, in particular how to collect troubleshooting information, will also be appreciated. My testers reported receiving "the application was not installed because the error message is unknown (0xE8008016)", which does not tell me anything that I may have done wrong.

I learned this hard way when I went through the build cycle, thinking that I saved and checked the correct .plist with get-task-allow unchecked.
