PHP / C / C ++ - What does your initial comment block look like in your programs?

I am in a large project and have not actually made any comments in the headers of my program yet. I just started using SVN for a project the other day. I want to add the SVN $ id $ tag, but have not resolved any standard comments.

So my question for everyone here is, what does your initial comment block look like in all your programs?

i.e:

I personally use the same comments as javadoc ones

Any initial comment block is a space space if you have version tracking software.
