SharePoint application profiling best practices

When the SharePoint application you are working on has an unsatisfactory response time, how do you identify the causes and in what order? So far, I have not yet found any open source tools or commercial profiling specific to the SharePoint segment, so any pointer to tools or best practices will be appreciated.

PS. I know the profiling options for web parts, but I'm looking for something that spans the entire application.
