MySQL, Asterisk Dialplans, and Call Forwarding

Question

How do I get Asterisk to forward incoming calls based on the correspondence of the incoming call number to the forwarding number? Both numbers are stored in the MySQL database.

The solution I was looking for looked like this:

Sorry for the long code sample, but more than half of it is debugging code to help you tweak it.

I assume that your server already has a modern version of PHP (c /usr/bin/php) with the PDO library and that you have a database table with a name fwd_tablewith columns caller_idand destination.
