The download form does not work in Firefox 3 with Mac OS X?

Today I ran into this strange problem with a user using Mac OS X. This user has always had a failed download. The form uses the usual "input type = file". A user can download from any browser other than Firefox 3 to their Mac. Only this particular user has seen this error.

Obviously, the problem is only with this particular user.
