How do charities rate the use of donated CPUs?

I saw from other questions how to measure the use of your own processor. But how does this happen in a distributed way? For instance. what if millions of computers around the world donate processor power (say) to cancer research. I have seen this a lot: often there is a table of prestigious leagues about who donated most of the processor power. How is it really measured? In particular, is it easy for someone to fake how much processor power they actually donated?
