How to insert a dynamic form as an activity or resource between sections on a moodle course?

We tried to insert form elements through quizzes in moodle, but when it is difficult to handle, when we show on the front side.

Please indicate how to insert a dynamic form as an activity or resource between sections?

Here is the actual thread we wanted to implement (e.g. wordpress):

• Create the form as we want.
• How to insert a form identifier [in the form of a short code or a similar method] into the section action.

Is it possible to implement as described above?
