How changes are changing in DocumentDB

I have a question regarding the _ts field in documentdb. How is this determined? I understand that when a document is added / changed, it gets a new _ts. Is it likely that two documents have the same _y? If so, will this only happen if these documents are added at the exact time (in milliseconds).

_ts - This is a system property that indicates when the document was last updated (for example, create or replace).

_tsrepresented as a POSIX value or time time . In other words, this is the number of seconds (not milliseconds) elapsed since 00:00:00 (UTC), January 1, 1970.
