VS 2015 dynamic menu commands do not work as advertised

I am trying to create dynamic menu items in the VS 2015 extension. The best I can do is leave the placeholder command visible and active. Here is the smallest example I could create, starting with the VS wizards (sorry for the length, but this is really the smallest example I could create):
