How to edit data in mysql via modal

I had errors with this code. I am new to downloading and using modals for things. I used to run child forms for all of my “bizarre” forms, but now I'm making the transition to modal. This is my code for capturing modals:

The problem is that I keep getting my "Error!". message in a modal field, not something else. The goal I'm trying to do is to pull data from an identifier that is stored in my database. I think the problem is that it does not capture the identifier when I click the edit button on the user. Honestly, I do not know how I can edit the identifier through modal. Help would be greatly appreciated. thank
