Why is there a gear icon and not the logo of my application on the screen for permission to use iOS Facebook?

I request "public_profile", "email", "user_friends", "user_birthday", "user_location" information from my iPhone application users and in the settings of my Facebook application, I configured my application with a 1024 x 1024 logo. When the application asks for permission in the first times, my application logo is displayed on the Facebook login confirmation screen. In subsequent requests for permission in the SDK, a confirmation page is displayed on the page, as shown here, but now there is an icon for this gear, not my application’s icon and user profile picture.

In my other application, this does not happen, and I can always see the application icon and user profile photo. I compared their settings, but could not understand the difference. Something is missing me, but I cannot understand.

I am using Facebook-iOS-SDK v3.14.

Facebook seems to have assumed this behavior after the release of the latest SDK.
