Sublime Text: multiple selection in selected

Question

I want to make multiple selections and edit within the specified selection in Sublime Text 2. The following screenshot illustrates the situation:

When I select the "item" and press Alt + F3 (on Windows), I can edit all instances of the item at the same time. However, what I would like to do is to be able to edit all instances of an element only in the container, leaving the element in the container unchanged. At first I tried to select a specific area, but as soon as I try to select an “element”, my first choice will disappear.

Can this be achieved in Sublime Text 2?
