Unclear Apache and Phusion Passenger bug in unsuccessful Rails applications - how to solve this problem without an answer?

I use Apache 2.4 along with Phusion Passenger 4.0.5, and for some reason I see errors that occur from time to time for Rails applications without any real rhyme or reason. When I look through the Apache error logs, I only see a very foggy error:

[ 2013-07-01 10:56:01.0046 26104/7ff716d317c0 apache2/Hooks.cpp:757 ]: Unexpected error in mod_passenger: Cannot connect to the helper agent Backtrace: (empty) 

Any way to get more words from Apache and Passenger to ease troubleshooting here?

This error message is displayed when HelperAgent fails. The code in Apache will wait for Watchdog to restart HelperAgent, after which the connection will be restored. If something goes wrong, you will receive this message. You should look for earlier posts. Are there any messages from earlier that can tell you why HelperAgent crashed in the first place or why it could not restart? If you look at the statistics of passive memory, do you see PassengerHelperAgent and PassengerWatchdog?
