Error in Cocoa document-based application with document window icon pop-up menu

I have a document-based application that supports versions and autosave. When you click the proxy icon, a pop-up menu appears, as in any other document application:

Have you noticed this weird NSMenuItem element? He should not have looked like that. For example, in Quartz Composer, it looks like this:

I don’t know why this is happening since I am not doing anything with the window title or icon. There is not even an API to configure this pop-up menu.
It would be great if someone could help me get this corrected, or at least an idea of ​​what might be causing this behavior.
